What is community health?

The term "community health" refers to the overall well-being of a population, and includes a variety of factors, including social determinants of health and health disparities. Community Health Workers (CHWs) serve as liaisons between community members and health or social services, and improve patient health literacy. Community health services combine health and wellness resources and provide health education opportunities.

Our community care team is made up of diverse and compassionate individuals to provide person-centered care and support every step of the way.
​Our team includes:

• Community Health Workers (CHWs) to assist patients in accessing the care and resources they need.
• Patient Health Navigators (PHNs) to help patients find pathways to resources.
• HIV Testing & Prevention Counselors (HIV-TPCs) to perform HIV testing and education to support patients in making informed decisions about their sexual health and available options.
• Smoking Cessation Support Coaches certified by the Respiratory Health Association.
• A certified Phlebotomist (CPT)
• A Psychiatric Technician (NCPT) to support patients in taking the next steps in their mental health goals.
• Behavioral Health Providers (LCSW, MHPs) to provide counseling services.

• A licensed Massage Therapist (LMT) to provide massage services (by appointment) to reduce stress and improve overall wellness.
